Mechanical Comparison between Fenestrated Endograft and Physician-Made Fenestrations

چکیده
Introduction: A fenestrated endograft (FE) is the first-line endovascular option for juxta and pararenal abdominal aortic aneurysms. physician-modified stent-graft (PMSG) laser in situ fenestration (LISF) have emerged to circumvent manufacturing delays, anatomic standards, procedure’s cost raised by FE. The objective was compare different fenestrations from a mechanical point of view. Methods: In total, five Zenith Cook (Cook Medical, Bloomington, IN, USA) Anaconda (Terumo Company, Inchinnan, Scotland, UK) were included this study. Laser ISF PMSG created on TX2 polyethylene terephthalate (PET) cover material USA). LISFs fifty-five created. All reached an 8 mm diameter. Radial extension tests then performed identify differences behavior between designs. branch pull-out force measured test stability assembling with calibrated branch. Fatigue devices assess long-term outcomes oversized 9 Results: results revealed that at over 2 oversizing, highest average radial strength 33.4 ± 6.9 N fenestration. higher custom-made fenestrations, including both (9.5 4.7 4.49 0.28 N). comparison LISF double loop highlighted value compared (3.96 1.86 vs. 2.7 0.82; p= 0.018). diameter varied mm. As pin caliber inserted mm, one could consider all underwent “elastic recoil” after cycling. largest elastic recoil observed non-reinforced/OC (40%). 10% LISF. Conclusion: terms behavior, produced strength. Both be improved standardization creation protocol.

Surgeon-modified fenestrated endograft to treat ruptured juxtarenal aneurysm.
IMPORTANCE No endovascular devices are commercially available in the United States to treat high-surgical risk patients with aneurysms extending to visceral arteries. Treatment options are even further limited for symptomatic patients in need of urgent treatment. OBJECTIVE To describe a successful urgent endovascular repair of a juxtarenal abdominal aortic aneurysm with contained rupture. D...

PURPOSE To describe a precannulated fenestrated endograft system utilizing externalized guidewires to facilitate aortic arch endovascular repair and to report its use in 2 patients with challenging anatomy. TECHNIQUE For distal arch repair, a fenestration for the left subclavian artery (LSA) is made onsite in a standard thoracic endograft tailored to the patient anatomy; it is precannulated w...

Explantation of an Infected Fenestrated Abdominal Endograft with Autologous Venous Reconstruction
INTRODUCTION Infectious complications after FEVAR cause significant problems, with radical surgery considered to be the last resort for treatment. CASE REPORT A 72 year old man presented with infection 1 month after FEVAR. Conservative therapy with percutaneous abscess drainage and antibiotics suppressed the infection for 10 months; however, when new peri-aortic abscesses developed, the patie...
